Care Manager Complex Social Worker (LCSW)

Job DescriptionThe Complex Care Manager collaborates with members of the healthcare team, community resources, and with the patient/patient family to promote optimal outcomes for the patient across the continuum of care.
The Complex Care Manager maintains a smaller caseload that requires increased attention and intensive care coordination due to complex social dynamics, legal involvement and connection with DSS/DDA.
The Complex Care Manager works in partnership with the Care Management Manager to provide oversight of daily departmental workflows and case assignments while assisting the care management team in navigating through barriers that may be preventing the safe disposition of patients.
The role assists with staff education on Joint Commission standards of care ensuring the Care Management department meets compliance metrics related to assessment, discharge appeals, freedom of choice, and abuse/neglect.
This position is goal-oriented with a high level of accountability correlated to the continuum of care, quality of care and patient satisfaction.
This position is regarded as essential to the success of the care coordination of this patient population with a high degree of professionalism and expertise required.

QualificationsQualificationsEducation:
Graduate of an accredited college with a degree in social work and current licensure in the State of Maryland.
Master's degree with LCSW certification required or graduate from an accredited School of Nursing or college with current nursing licensure in the State of Maryland.
BSN or Master prepared preferred.
Experience:
Minimum of 3-5 years of relevant clinical hospital experience required as applicable to the position needs.
Ongoing education, certification, and self-development to remain current with industry standards and business objectives related to hospital case management.

Exposed to frequent interruptions and multiple demands.

Physical RequirementsMust be able to sit for short periods; standing, walking, stooping, and bending is required.
Frequent listening, telephone communication, visual acuity to read medical records, department literature, data reports and video viewing display monitors is required.
Lifting of less than 25 pounds with frequent handling of equipment and supplies.
